Brookhaven Hospice is seeking a Per Diem Licensed Nursing Assistant (LNA) to join our compassionate team, providing hands-on care that brings comfort and dignity to patients at the end of life. Working closely with our nursing team, you’ll ensure each patient’s physical, emotional, and personal needs are met with respect, empathy, and professionalism.

The schedule is Per Diem with coverage in Windham, Salem, Nashua and Derry.

Qualifications

• High School Diploma or GED required.
• Holds current unencumbered LNA license in the state working as an LNA.
• Minimum one year experience as an LNA in acute care or LTC; hospice experience a plus.

Responsibilities

• Provide Personalized Care: follow the care plan to ensure patient comfort and well-being and attend promptly to requests from patients and families.
• Assist with Personal Care: aid with bathing, oral hygiene, changing bedding, clothing, and shampooing to support comfort and dignity.
• Be Dedicated to Customer Service: deliver exceptional service to ensure patient satisfaction and valued care.
• Provide Support for Families: guide and support families in caring for loved ones during end of life.
• Maintain Accurate Documentation: document all care details in the home and agency records.

Qualifications

• High School Diploma or G.E.D. required.
• Holds current unencumbered license in the state working as a LNA.
• Minimum one (1) year experience as a LNA. in an acute care or LTC setting. Hospice exp a plus!
• Current driver's license, vehicle insurance, and access to dependable transportation.

$24.00 - $26.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
