Job summary

Brookhaven Hospice is seeking a Per Diem licensed nursing assistant (LNA) to provide compassionate, hands‑on end‑of‑life care. Work closely with the nursing team to meet the physical, emotional, and personal needs of patients in Windham, Salem, Nashua, and Derry.

Qualifications

• High School Diploma or GED required.
• Holds current unencumbered LNA license in the state of practice.
• Minimum one year experience as an LNA in acute care or LTC; hospice experience a plus.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Provide personalized care following the plan of care to support patient comfort and well‑being.
• Assist with personal care needs including bathing, oral hygiene, bedding, clothing, and shampooing.
• Demonstrate exceptional customer service and support to patients and families.
• Support families by sharing knowledge and guidance in caring for terminally ill loved ones.
• Maintain accurate documentation in patient records and regulatory standards.

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well‑being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.


• Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity.

Qualifications

• High School Diploma or G.E.D. required.


• Holds current unencumbered license in the state working as a LNA.


• Minimum one (1) year experience as a LNA. in an acute care or LTC setting. Hospice exp a plus!


• Current driver's license, vehicle insurance, and access to dependable transportation.

$20.00 - $24.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
