Education Requirements

• Valid Vermont Nursing Assistant License

Qualifications

• High School Diploma or GED
• Valid Driver's License and Auto Insurance
• Open to experienced and newly graduated LNAs
• Must be at least 18 years old
• Weekend and holiday shifts required

Responsibilities

• Perform patient care under supervision of RN or licensed Therapist, following individualized care plans.
• Gather and report patient and family data promptly.
• Communicate effectively with patients and families.
• Assist with interventions and procedures as directed by RN or Therapist to ensure safe, quality care.
• Report patient and family updates to RN or Therapist.
• Advocate for patient needs to ensure personalized care.
• Perform activities of daily living, vital signs, ambulation, exercises, etc.
• Serve as Hospice Aide and Personal Care Attendant as directed.
• Report changes in skin conditions promptly to RN or Case Manager.
