Job summary

Franklin VNA & Hospice is seeking a compassionate Licensed Nursing Assistant to provide basic patient care under the direction of the RN and Clinical & Staff Development Manager. Tasks include recording vital signs, assisting with daily living activities, and light Homemaker duties as needed.

• High School diploma or equivalent and graduation from an accredited Licensed Nursing Assistant program.
• Current Certification/Licensure for State of New Hampshire is required.
• Minimum of 6 months of experience in an LNA or related role required.

Responsibilities

• Provide basic patient care under direction of the RN and staff manager.
• Take and record vital statistics, assist with daily living activities and homemaker duties as needed.
• Report changes in patient condition to nursing staff and maintain safe environment.
• Record health information and assist with care coordination.
• Travel as needed within central New Hampshire.

Provide basic patient care under direction of the RN and Clinical & Staff Development Manager. Perform duties such as taking and recording vital statistics, feed, bathe, dress, groom, move patients and change linens. May also perform light Homemaker duties.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

• Adhere to patient/client LNA care plan as set up the while maintaining a healthy and safe environment.
• Reports and documents patient/client’s needs and responses to the Nursing supervisor.
• Reports patient/client’s change in condition or situations immediately.
• Provide physical support to assist patients to perform daily living activities, such as getting out of bed, bathing, dressing, using the toilet, standing, walking, or exercising.
• Turn or reposition bedridden patients.
• Record vital statistics or other health information.
• Review patients' dietary restrictions, food allergies, and preferences to ensure patient receives appropriate diet.
• Measure and record food and liquid intake or urinary and fecal output, reporting changes to medical or nursing staff.
• Monitor patients to detect health problems and advise Nursing supervisor.
• Perform light housekeeping and Homemaker duties deemed acceptable by the agency in conjunction with or in the absence of the agency Homemaker.
• Perform other duties assigned.

Requirements

EDUCATION AND EXPERIENCE

• High School diploma or equivalent and graduation from an accredited Licensed Nursing Assist program.
• Current Certification/Licensure for State of New Hampshire is required.
• Minimum of 6 months of experience in an LNA or related role required.

• Ability to wear protective clothing such as mask, glove, head cover, lead apron, safety glasses, safety shoes, etc.
• Ability to travel required. Travel throughout central New Hampshire is required.
• Current drivers’ license is required.
• Proof of automobile insurance is required.
• Ability to pass a motor vehicle background screening is required.

PHYSICAL DEMANDS/WORK ENVIRONMENT

• Job requires physical activities including but not limited to; standing for extended periods of time, bending, lifting, reaching, kneeling, pushing, pulling, crouching, crawling, fingering, grasping, feeling, talking and hearing.
• Job requires moderate physical work including the ability to exert up to 50 lbs. of force occasionally, 20 lbs. regularly.
• Job requires close visual acuity including the ability to see at or within arm’s reach.
• Job is subject to inside environmental conditions; low noise level.
