Job summary

Woodward Skilled Nursing & Therapy is seeking a licensed nurse (RN or LPN) to join our patient‑centered care team in Woodward, OK. You will administer medications, monitor residents, and collaborate with physicians and families to ensure high‑quality care.

Qualifications

• Current Oklahoma LPN or RN license.
• 1+ year of experience in a Skilled Nursing or Long Term Care setting.

Responsibilities

• Obtain and administer prescribed medications and treatments.
• Observe residents, document changes, and notify physicians of significant events.
• Record vital signs and monitor health status.
• Communicate with families and physicians about changes in health or medications.
• Assist with care plan updates and ADLs, updating nursing charts as needed.
• Provide injections and/or enemas when required.
• Collect specimens and monitor catheters.
• Deliver wound care.

Responsibilities and Purpose

• Obtain and administer prescribed medications and treatment in accordance with approved nursing techniques.
• Observe residents and record significant conditions and reactions. Notify appropriate parties of residents’ change in condition and reaction to drugs, treatments and significant incidents.
• Take vital signs to detect deviations from normal and assess condition of residents.
• Keep residents’ family and physician informed of any changes in resident behavior, medications or conditions of health.
• Modify residents’ care plan, care directives, Activities of Daily Living (ADL) and Nurse Aide flow sheets in conjunction with residents’ changes in condition.
• Prepare and administer injections and/or enemas.
• Collect samples and monitor catheters.
• Provide wound care.

Required Experience and Education

• Current and unencumbered Oklahoma Licensed Practical Nursing (LPN) License OR Oklahoma Registered Nurse (RN) License.
• 1+ years of experience in a Skilled Nursing or Long Term Care environment.
