Job summary

Commonwealth Senior Living in Haddam, CT is seeking a PRN Licensed Nurse (LPN/RN) to support residents in Assisted Living and Memory Care. You will conduct assessments, manage medications, implement care plans, and document care under the Resident Care Director.

We offer 3-4 shifts per month with flexibility for vacations. The role requires an unencumbered RN or LPN license, CPR/First Aid certification, and at least one year of nursing experience in geriatric or related settings.

Qualifications

  • Unencumbered RN or LPN license required.
  • CPR and First Aid certification, or willingness to obtain.
  • Experience in geriatric, psychiatric, or rehabilitation settings preferred.
  • Strong communication and documentation skills.

Responsibilities

  • Prioritize resident and family satisfaction and treat everyone as a valued customer.
  • Ensure compliance with Federal and State policies governing resident care.
  • Administer medications and treatments with thorough record-keeping.
  • Provide direct nursing care and supervise nursing assistants.
  • Report changes in residents' conditions promptly to the Resident Care Director.

Job description

Pay rate: $34 per hour.

The Licensed Nurse (LPN/RN) plays a pivotal role in supporting residents within Assisted Living and Memory Care settings. They conduct assessments, devise care plans, manage medications and treatments, implement care strategies, evaluate resident progress, and meticulously document all care services. Utilizing the nursing process, they ensure adherence to the Nurse Practice Act and regulatory standards, working under the guidance of the Resident Care Director while upholding Commonwealth Senior Living's principles. We are seeking to fill a PRN position for each shift, could expect 3-4 shifts per month plus covering for upcoming vacations.

Qualifications
  • Holds an unencumbered state license as a Registered Nurse or Licensed Practical Nurse.
  • CPR and first aid certification, or willingness to obtain.

Primary Responsibilities
  • Prioritize resident and family satisfaction by treating everyone as a valued customer.
  • Ensure compliance with Federal and State policies governing resident care.
  • Assist in monitoring and maintaining the Medication Management Program's safety and efficacy.
  • Administer medications and treatments accurately, maintaining detailed records.
  • Provide direct nursing care and supervise nursing assistants.
  • Promptly report changes in residents' conditions to the Resident Care Director.
  • Safely assist residents with mobility and transfers.
  • Responsively address inquiries from residents, visitors, and staff.
  • Support residents based on their unique care plans.
  • Foster effective communication through documentation and daily updates.
  • Handle controlled substances securely, reporting any discrepancies.
  • Maintain equipment and supplies to ensure resident safety.
  • Uphold residents' rights and promptly report any concerns.
  • Adhere to safety and infection control protocols.
  • Perform additional tasks as assigned by the Resident Care Director or Assistant Resident Care Director.
  • Delegate tasks appropriately while maintaining overall accountability.
  • Attend regularly scheduled meetings, conferences, and training sessions to maintain professional licensure.
  • Maintain regular communication with the Resident Care Director.
  • Ensure a safe working environment through accident prevention and equipment maintenance.
