Shadow Mountain Memory Care, located in Phoenix, AZ, is seeking a professional and dedicated Licensed Nurse to ensure quality nursing care is provided to the residents in our community.

Licensed Nurse LPN X2 (PRN and night evening shift needed Sunday to Thursday)

Responsibilities of the Licensed Nurse

  • Exhibits leadership qualities and communicates professionally with employees, residents, family members, responsible parties, and other healthcare professionals.
  • In conjunction with the Health and Wellness Director, establish the appropriate level of care for the resident, based on individualized care needs.
  • Perform assessments of residents and identify needs related to the physiological, psychological, environmental, and social needs upon move in and when a change of condition has occurred.
  • Monitor the 24-hour communications and alert system for compliance. Appropriately and timely assess residents who have been put on alert status. Remove residents from alert after appropriate assessment of their condition.
  • Collect and analyze pertinent health-related information.
  • Investigate and document incidents and behavior changes that require a licensed nurse assessment.
  • Implement plan of care formulated by physician and Health and Wellness Director.
  • Utilize safe and appropriate techniques/nursing skills in providing resident care.
  • Communicate with staff regarding changes in resident’s care needs.
  • Participate in staff development activities.
  • Become knowledgeable of state regulations governing assisted living communities.
  • Assist in daily requirements and tasks as requested.
  • Ensure residents are provided privacy, respect, and dignity.
  • Follow standard precautions and infection control procedures.
  • Complete state-required training upon hire and annually.

Required Experience

  • Possess, at a minimum, a high school diploma or equivalent.
  • A current Licensed Nurse in the state of Arizona is required.
  • A minimum of one (1) year prior nursing experience in a senior living community or related business preferred.
  • Basic computer skills.
  • Must be able to speak, read, and write in the English language.
  • Must obtain valid Continuing Education credits as required by state regulations.

Job Type
