Admissions Coordinator-Home Care

We're culturally sensitive, and many of our staff come from within the community that they serve. Operating 24 hours, 7 days a week, we offer reliable, dignified care in your own home and specialize in health services, personal care, and champion services.

We are seeking a Licensed Nurse (RN/LPN) to assume the role of Admissions Coordinator to serve as the primary point of contact for prospective patients and their families, guiding them through the admissions process with empathy, professionalism, and expertise. This position plays a pivotal role in ensuring the smooth admission process for patients onto our CHHA program.

Key Tasks for this role include;

  • Coordinate the patient referral and approval process in an efficient manner
  • Assign initial ICD 10 code to initiate authorization process for all referrals for HMO patients
  • Establish & maintain relationships with local hospitals, physicians & community organizations for the purpose of generating consistent admissions to the facility
  • Understand the admissions process including policy, procedures, assessments & paperwork requirements
  • Communicate special needs of new admissions to staff to ensure a smooth transition
  • Maintain knowledge of regulations governing Medicare/Medicaid/Managed Care reimbursement processes
  • Conduct thorough assessments of prospective patients to determine their care needs and eligibility for admission, collaborating with healthcare professionals as needed
  • Collaborate with marketing teams to develo
  • p strategies for promoting the CHHA and attracting new patients
  • Work closely with the CHHA's interdisciplinary team, including nursing staff, social workers, and administrators, to coordinate admissions and provide comprehensive care to patients

What We're Looking For

  • Active RN/LPN License in New York State
  • Degree from an accredited Nursing program
  • Experience in health care admissions or discharge planning environment
  • Hospital discharge planner experience a plus
  • Knowledge of Home Care billing - Medicare, Medicaid and third-party payor
  • Knowledge of ICD - 10 and CPT - 4 coding and medical terminology
  • Excellent communication skills
