Job summary

Post Acute in Portland, OR is seeking an experienced licensed nurse (LPN or RN) to support our team of Physician providers in post-acute settings. This role works directly with a physician to manage day-to-day medical tasks and clinical practice management within local facilities.

You will initiate and maintain patient records, schedule visits and referrals, take medical histories, coordinate care with community partners, and assist providers during exams and visits.

Qualifications

• Valid nursing license in Oregon.
• Minimum two years in hospital, skilled nursing, or clinic setting.
• Ability to travel with physician within local areas.
• Knowledge of health and patient care regulations.
• Strong ethics and teamwork.
