Job summary

Deaconess HomeCare, a part of LHC Group and Optum family, provides home health, hospice and community-based services to patients in home settings. The Social Worker will address significant social and emotional factors impacting health and support coping strategies.

The role partners with physicians and the interdisciplinary team to develop and maintain a comprehensive Plan of Care, linking patients with community resources to ensure personalized, compassionate care.

Qualifications

• Master's Degree in Social Work from an accredited program.
• Licensed Social Worker (LSW) in the state of residence.
• Current driver's license and access to a vehicle or reliable transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Assist patients and caregivers in understanding social, emotional, environmental factors related to health.
• Contribute to development of a comprehensive, integrated Plan of Care with the care team.
• Instruct on community resources available to assist patients as needed.
