Job summary

Amedisys, part of the Optum family of businesses, seeks a Medical Social Worker to provide psychosocial support, assessment, and care coordination for patients and families in home health. You will collaborate with an interdisciplinary team to link patients with community resources and counseling services, guiding transitions with empathy.

The role requires an MSW or Doctorate in Social Work, LMSW/LCSW licensure, CPR certification, and at least 1 year of healthcare social work experience.

Qualifications

• Master's or Doctorate in social work from a CSWE-accredited program.
• Active LMSW or LCSW licensure.
• Current CPR certification.
• 1+ years of social work experience in a healthcare setting.
• Meets state and federal requirements.

Responsibilities

• Provide psychosocial support, assessment, and care coordination to patients and families in home care.
• Collaborate with interdisciplinary team to connect patients with community resources and counseling.
• Assist with planning, transfer and discharge functions.
• Educate patients and families on social work interventions and treatment plans.
• Identify risks to patient health and coordinate appropriate interventions with the healthcare team.
• Perform other duties as assigned.
