Reviews assessments at time of admission, determines clinical interventions, facilitates therapy and provides education for patients and their families. Facilitates treatment planning and manages each case. Responsible for development of transition/discharge plan. Communicates effectively and respectfully with others, including interdisciplinary teams and all other contacts. Functions as an interdisciplinary team member.

The posted pay range reflects a 40-hour workweek (1.0 FTE) and aligns with the minimum and maximum pay rates specified in the union agreement at the time of job posting. The pay rate for an individual within this range may vary based on factors such as FTE, skills, knowledge, relevant education, andqualifying experience as detailed in the contract.
