Job summary

Sea Mar Community Health Centers is seeking an Integrated Mental Health Therapist Licensed to work within a medical setting. The role provides diagnostic assessments, referrals for psychiatric services, brief counseling, and case management for adults, children, and families in Washington state.

The IMHT Licensed collaborates with the Clinical Care Team, participates in treatment planning, crisis stabilization, and community referrals, while documenting care and maintaining quality measures

Qualifications

• Ability to provide diagnostic assessments for patients in the program.
• Knowledge of mental health, substance abuse, and chronic disease management.
• Experience coordinating with interdisciplinary care teams and community providers.

Responsibilities

• Provide diagnostic assessments and referrals for psychiatric services.
• Deliver brief, strength-based counseling and crisis stabilization when needed.
• Coordinate treatment plans with BH Supervisor and care team to meet patient goals.
• Document encounters in the electronic health record within 24 hours.
• Conduct PHQ9 and other screenings; support treatment referrals as required.
