Job summary

Recreate Behavioral Health in Ohio seeks a qualified Mental Health Therapist to work with individuals facing substance use disorders, providing individual, group, and family therapy within a compassionate treatment team. You will also help formulate the Master Treatment Plan and coordinate discharge planning.

Responsibilities include crisis intervention, psychosocial assessments, and presenting findings to the team. Licensure in Ohio and HIPAA compliance are required.

Qualifications

• Master's degree in social work, psychology or related human services field.
• Current license as an LPC, LPCC, LSW, LISW or LMFT in Ohio.

Responsibilities

• Maintain open communication with facility staff, attending clinicians, physicians, clients, and families.
• Liaison with community agencies, vendors, and payers of health services.
• Review assessments from referring facilities and note problems/issues requiring attention.
• Gather diagnostic information through psychosocial assessment interview.
• Develop an Integrated Summary identifying problem areas affecting treatment planning.
• Create a comprehensive problem list reflecting client issues from assessments.
• Present psychosocial and diagnostic information to the treatment team clearly.
