Lumera Healthcare is seeking a compassionate, motivated, and skilled Licensed Mental Health Therapist to join our growing outpatient behavioral health team. Key Responsibilities
The Licensed Mental Health Therapist will:
- Assist with clinical intakes and assessments to help establish medical necessity and identify appropriate services.
- Participate in the treatment planning process from intake through discharge based on each client's diagnosis, needs, goals, and progress.
- Provide individual, family, and group therapy as clinically appropriate.
- Develop, implement, review, and update individualized treatment plans.
- Maintain complete, accurate, and timely clinical documentation in accordance with organizational, payer, regulatory, and professional standards.
- Ensure services provided are medically necessary, evidence-based, client-centered, and within the ethical and legal scope of practice.
- Monitor client progress and adjust treatment interventions as clinically appropriate.
- Coordinate care with physicians, case managers, community resources, and other service providers when appropriate.
- Participate in case consultations, clinical meetings, and interdisciplinary collaboration as indicated.
- Maintain professional boundaries and adhere to all confidentiality, HIPAA, ethical, and compliance requirements.
- Support continuity of care and appropriate discharge planning for clients.

Licensure & Qualifications
Candidates must hold an active or eligible Kentucky license in an applicable behavioral health discipline, including:
- Licensed Clinical Social Worker (LCSW)
- Certified Social Worker (CSW)
- Licensed Marriage and Family Therapist (LMFT)
- Licensed Professional Clinical Counselor (LPCC)
- Licensed Professional Counselor Associate (LPCA), when appropriate for the position
- Licensed Psychological Practitioner (LPP), when applicable
- Licensed Psychologist (PhD/PsyD)
- Other qualifying Kentucky behavioral health licensure or credentials appropriate to the services provided
Previous experience in a behavioral health, clinical counseling, social work, or outpatient setting is preferred.
