What You’ll Do
Provide Patient-Centered Clinical Care
- Deliver individual, family, and group therapy to long-term care residents
- Complete clinical assessments, psychosocial evaluations, screenings, and individualized treatment plans
- Support patients experiencing depression, anxiety, grief, trauma, adjustment to illness, substance use concerns, behavioral disturbances, and other mental health conditions
- Provide brief, goal-oriented interventions appropriate for older adults and medically complex patients
- Apply evidence-based behavioral health practices to improve patients’ emotional well-being and quality of life
Collaborate with an Interdisciplinary Team
- Partner with nursing, social services, rehabilitation, and medical providers
- Participate in care planning, case collaboration, and facility meetings as needed
- Communicate treatment recommendations clearly and professionally
- Serve as a trusted behavioral health resource for facility staff and leadership
Maintain Documentation and Compliance
- Complete accurate and timely clinical documentation using a secure electronic documentation platform
- Follow professional, ethical, confidentiality, and regulatory requirements
- Maintain high standards for treatment planning, progress notes, and continuity of care
Qualifications
Required
- Active and unrestricted Licensed Mental Health Counselor license
- Valid driver’s license and reliable transportation
- Strong written, verbal, and interpersonal communication skills
- Ability to work independently while collaborating with an interdisciplinary team
- Commitment to timely documentation and high-quality patient care
- Willingness to provide on-site services in long-term care facilities within the local area
Preferred
- Experience working with older adults, geriatric patients, or medically complex individuals
- Experience in long-term care, skilled nursing, rehabilitation, healthcare, or integrated behavioral health
- Confidence providing therapy and behavioral interventions in a facility-based environment
Previous long-term care or geriatric experience is helpful but is not required for a qualified and motivated clinician.
