Base pay range

$62.00/hr - $67.00/hr

We're looking for Licensed Master Social Worker for our client with a leading hospital in Bronx, NY.

Please find below details of the position.

Position: Licensed Master Social Worker - BH

Location: Bronx, NY.

Duration: 3 months contract with extension possibilities.

Shift: 9am-5pm Mon-Fri

Pay - $62/hr

Job Duties:

1. Complete psychiatric and psychosocial assessments on all new admissions to determine patient healthcare needs.

2. Contact collaterals for information gathering and coordination of care. Arrange family and collateral meetings when appropriate.

3. Participate and contribute to clinical discussions with multi-disciplinary team to plan for patients’ next level of care.

4. Formulate and initiate appropriate discharge planning options. Responsible for all documentation that will facilitate patient’s discharge.

5. Liaise with managed care companies to obtain authorizations for hospitalizations and transportation when necessary.

6. Facilitate and organize outpatient care with other providers if appropriate.

7. Maintain statistics when necessary. Always complete clinical documentation in a timely, appropriate and accurate manner.

8. Support the service and social work department in meeting and maintaining regulatory requirements and completing surveys.

9. Lead or co-lead psychotherapeutic groups when indicated.

10. Attend supervision and departmental meetings as required.

11. Rotation/Coverage of different services is required as needed.

Experience required:

1 year of experience required.

Education Required:

Licensed Master Social Worker (000553)

Valid Licensed Master of Social Work (LMSW) license issued by NYSED, Primary Source Verification
