Qualifications

Responsibilities

• Conduct psychosocial assessments to evaluate patient needs, barriers to care, and support systems.

• Assist with discharge planning, including care coordination, placement support, and referrals to post-acute or community-based resources.
• Provide crisis intervention, emotional support, and patient/family counseling as appropriate.
• Coordinate with medical staff, case managers, and outside providers to facilitate safe care transitions.
• Maintain timely and accurate documentation within electronic medical record systems.
• Support patients navigating complex medical, social, and behavioral health needs during hospitalization.
• Help identify and connect patients to community resources, benefits, and support services following discharge.
