Job summary

PrimeCare Medical, Inc. is seeking an Alcohol and Substance Abuse Counselor to work in the Montgomery County Correctional Facility in Eagleville, PA. This full-time role requires certification as CADC/CAADC and/or licensed clinician (LSW/LCSW or LPC).

The schedule is Monday through Friday, 8:00 am–4:30 pm. The counselor will monitor, coordinate, and implement MAT in accordance with standards and PCM policies, serving as a liaison between medical staff and facility personnel.

Qualifications

• Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or related Human Services discipline with mental health coursework.

Responsibilities

• Clinical monitoring, coordinating and implementing MAT per NCCHC/ACA standards and PCM policies.
• Serve as MAT liaison between medical unit and facility staff.
• Engage inmates/patients in therapeutic interventions for substance abuse.
• Assist with suicide prevention and clinical risk assessment.
• Document treatment responses and participate in interdisciplinary team meetings.
