FutureCare Health & Management is seeking a Director of Social Work to advocate for residents and improve access to care within our facilities. You will provide crisis intervention support, clinical supervision, and lead the social work team to ensure high standards of care.

Qualifications

• Bachelor’s degree in Social Work is required; Master’s preferred.
• Maryland Social Work license is required.
• Experience in skilled nursing and long-term care is required.
• Management experience in an interdisciplinary setting is preferred.
• Ability to lead and develop the department and staff.

Responsibilities

• Coordinate and supervise medically related social services for residents.
• Supervise programs designated to the Social Work Department.
• Participate in daily interdisciplinary team meetings.
• Assist in meeting psychosocial needs of residents and families.
• Address advance directives and determine medical decision makers.
• Provide individual, family and group psychosocial services.
• Promote residents' rights and dignity.
• Assist in developing an interdisciplinary care plan.
• Provide support and grief counseling as indicated.
• Oversee discharge planning processes to other settings.
