Job summary

Piedmont Healthcare is seeking a Licensed Dietitian to provide evidence-based medical nutrition therapy across our care settings. You will assess patients’ nutritional status, develop individualized care plans, and collaborate with physicians, nurses, and care teams to support optimal outcomes.

You will educate patients and families, document in the EMR, and participate in quality and performance improvement, all within a collaborative, patient-centered culture at a respected non-profit health

• Medical nutrition therapy (MNT)
• Clinical nutrition assessment
• Care planning and documentation in EMR
• Nutrition counseling and patient education
• Interdisciplinary care collaboration
• Knowledge of chronic disease management (e.g., diabetes, renal, cardiac)
• Menu planning and therapeutic diets
• Anthropometric and biochemical data interpretation
• Regulatory and accreditation standards (Joint Commission, CMS)
• Time management and caseload prioritization

Responsibilities

• Conduct comprehensive nutrition assessments for patients across care settings
• Develop and implement individualized medical nutrition therapy plans
• Collaborate with physicians, nurses, and interdisciplinary teams on patient care
• Provide nutrition education and counseling to patients and families
• Monitor and document patient progress in the EMR
• Adjust nutrition plans based on clinical status, labs, and patient goals
• Participate in quality improvement and clinical best-practice initiatives
• Stay current with evidence-based nutrition guidelines and standards of care
• Support transitions of care with appropriate nutrition recommendations
• Contribute to a patient-centered, respectful, and inclusive care environment
