Job summary

Fairfield Community Health Center in Fairfield County, OH is seeking a licensed counselor or social worker to join our Behavioral Health team in a full-time role.

Qualifications

• Must meet licensure requirements as defined by the Ohio Counselor, Social Worker, and Marriage & Family Therapist Board.
• Experience with multi-need individuals and families.
• Broad knowledge of community service systems.
• Able to communicate effectively verbally and in writing.

Responsibilities

• Assesses strengths and needs of referred individuals and families.
• Provides and coordinates clinical and social services that are problem-focused.
• Performs utilization review and quality assurance activities.
• Maintains documentation, participates in program evaluation and meetings, and learns community resources.
• Meets billing productivity requirements.
• Other duties as assigned.
