Responsibilities

What you will do: (responsibilities listed are included but not limited to)

• Perform comprehensive evaluations assessing social, emotional, and support needs of patients and their families
• Set measurable objectives in conjunction with the rehab team
• Assist physicians and health team members in understanding social and emotional factors related to health problems
• Instruct and counsel patients and families in coping with social and emotional responses related to illnesses
• Participate in care coordination activities and act as a resource to other health team members
• Identify and help patients and families utilize community resources to achieve measurable objectives

Qualifications

What you'll need:

• Master's Degree in Clinical Social Work (LCSW)
• Current Licensed Clinical Social Worker Licensure in the state of practice
• Minimum of one year experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or acute diagnoses
• CPR certification required
• Ability to lift 50 pounds
• Physical ability to move, lift, or transfer patients, sometimes up to 100 pounds following safety procedures
• Fine motor skills, visual acuity, and ability to walk/stand for extended periods
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and repositioning frequently
