Job Title: Contract Licensed Clinical Social Worker (LCSW) – Remote/Telehealth

Location: Remote (Must be licensed in California)

Position Overview:

Centered Care Wellness is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our telehealth team on a contract basis. This role focuses on providing high-quality mental health and social work support to vulnerable and underserved populations, including adults, seniors, and veterans. The ideal candidate will excel in remote patient care, be adept at assessing social determinants of health, and collaborate with interdisciplinary teams to support comprehensive patient care plans.

Key Responsibilities:

1. Provide counseling, crisis intervention, and emotional support to patients via telehealth platforms.
2. Conduct thorough assessments of social determinants of health and connect patients to appropriate community resources.
3. Develop and implement individualized care plans tailored to patient needs, with a focus on chronic disease management and transitional care.
4. Collaborate with healthcare providers, including nurse practitioners and physicians, to coordinate holistic patient care.
5. Offer guidance on coping strategies, stress management, and mental health education.
6. Support patients with navigating healthcare systems, insurance, and other social services.
7. Maintain timely, accurate, and confidential documentation in compliance with HIPAA standards.

Qualifications:

1. Master’s Degree in Social Work (MSW) from an accredited institution.
2. Current and active LCSW license in California (required).
3. Minimum of 2 years of experience in medical social work, mental health services, or related fields.
4. Passion for working with underserved and vulnerable populations, including seniors, veterans, and those with chronic conditions.
5. Experience with telehealth or virtual care delivery (preferred).
6. Bilingual proficiency in English and Spanish (preferred).
7. Excellent communication, organizational, and interpersonal skills.
