Rehab Without Walls® is seeking an experienced Licensed Clinical Social Worker (LCSW) to join our Home & Community–based Neurorehabilitation Program in San Francisco. This PRN opportunity offers meaningful flexibility while allowing you to deliver impactful, relationship–driven care in patients’ homes and communities.

PRN Caseload Details
- Each client is typically seen 4–6 hours per week.
- Schedule is created collaboratively with the client to support functional goals and real‑life routines.

Responsibilities
- Perform comprehensive evaluations, assessing the social, emotional, and support needs of patients and their families.
- Set measurable objectives that are formulated in conjunction with the rehab team.
- Assist the physician and other health team members in understanding the significant social and emotional factors related to the patient’s health problems.
- Instruct/counsel patients and families in treating and coping with social and emotional responses connected with illnesses.
- Participate in care coordination activities and act as a resource to other health team members in the identification and resolution of patient needs.
- Identify and assist the patient and/or family to utilize appropriate community resources to achieve measurable objectives.
Qualifications
- Master’s Degree in Clinical Social Work (LCSW).
- Current Licensed Clinical Social Worker Licensure in the state of practice.
- Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury, or acute diagnosis.
- Current BLS (Basic Life Support) First Aid and CPR certification required.
- Ability to lift 50 pounds.
- Ability to lift up to 100 pounds following safety procedures.
- Fine motor skills, visual acuity, and ability to walk/stand for extended periods.
- Ability to perform pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently.
Salary
USD $60.00 - $70.00 per hour.
