Job summary

BrightSpring Health Services is seeking an experienced Licensed Clinical Social Worker (LCSW) to join our Home & Community-based Neurorehabilitation program in the San Francisco area. This PRN role offers flexible scheduling and the opportunity to deliver counseling and support in patients' homes and communities.

The LCSW will work with a diverse interdisciplinary team to address physical, emotional, cognitive and social needs, helping patients regain independence and improve quality of life

Qualifications

• Master's degree in Clinical Social Work (LCSW) and current LCSW licensure.
• Minimum of 1 year rehab experience serving individuals with brain/spinal injuries or acute diagnoses.
• Current BLS First Aid and CPR certification required.
• Ability to lift 50 pounds; may involve lifting up to 100 pounds with safety procedures.

Responsibilities

• Performs comprehensive evaluations of social, emotional and support needs of patients and families.
• Sets measurable objectives with rehab team.
• Assists physicians and health team in understanding social and emotional factors affecting health problems.
• Instructs and counsels patients and families in coping with social and emotional aspects of illness.
• Participates in care coordination and acts as a resource to the health team.
• Identifies and connects patients and families to community resources to achieve objectives.

PRN Caseload Details:

• Each client is typically seen 4-6 hours per week
• Schedule created collaboratively with the client to support functional goals and real-life routines

Responsibilities

• Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families
• Sets measurable objectives that are formulated in conjunction with the rehab team
• Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient’s health problems
• Instructs/counsels' patients and families in treating and coping with social and emotional response connected with illnesses
• Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
• Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives

Qualifications

• Master's Degree in Clinical Social Work (LCSW)
• Current Licensed Clinical Social Worker Licensure in the state of practice
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
• Current BLS (Basic Life Support) First Aid and CPR certification required
• Ability to lift 50 pounds
• Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
• Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
