Job summary

COMMUNITY HEALTH PARTNERSHIP OF ILLINOIS seeks licensed Clinical Social Workers to provide comprehensive mental and substance use health services across CHP sites. You will deliver evidence‑based psychotherapy, conduct assessments, and develop individualized plans for diverse populations including children, adults, and seniors.

Responsibilities include crisis intervention, care coordination with healthcare teams, and maintaining HIPAA‑compliant documentation in EMR systems.

Qualifications

• LCSW license in state of practice.
• Experience in psychotherapy or behavioral health settings.
• Familiarity with ICD-9/ICD-10 coding.
• Ability to work with diverse populations.

Responsibilities

• Conduct comprehensive patient assessments and develop treatment plans.
• Provide individual, group, and family therapy using CBT, psychodynamic, ABA, and crisis intervention modalities.
• Coordinate care with healthcare teams and medication management when applicable.
• Maintain HIPAA-compliant documentation using EMR systems.
• Engage in clinical research activities to support quality improvement.
• Deliver crisis management and urgent mental health response.
• Offer targeted counseling such as Christian or addiction counseling as appropriate.
• Assist in case management to connect patients with community resources.
