Job summary

Collage Home and Community Rehabilitation Partners in San Diego, CA is seeking experienced Licensed Clinical Social Workers to provide patient-centered care in patients' homes and communities, with PRN assignments available for the duration of each stay.

You will work with interdisciplinary teams to develop individualized treatment plans, assess and counsel medically complex patients, and coordinate resources to support disability adjustment.

Qualifications

• Master's degree in Social Work (MSW) is required and LCSW licensure
• Excellent planning, organization, writing, and time management skills are required
• Ability to communicate with families, professionals, and co-workers is essential

We are looking for experienced Licensed Clinical Social Workers who want to provide truly patient-centered care in San Diego, CA, and surrounding areas- with PRN positions available for clinicians who will accept patients for the duration of their stay. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.
