Collage Home and Community Rehabilitation Partners in Oceanside, CA is seeking experienced Licensed Clinical Social Workers to provide patient-centered care in patients' homes and communities. PRN positions are available for clinicians who can accept patients for the duration of their stay.

Our interdisciplinary teams create integrated treatment plans and engage families and professionals to achieve functional goals.

Qualifications

• Completion of a Master's degree in Social Work and state licensure as an LCSW are required.
• Excellent planning, organization, writing, and time management skills are required.
• Ability to communicate with families, professionals, and co-workers is a must.

We are looking for experienced Licensed Clinical Social Workers who want to provide truly patient-centered care in Oceanside, CA and surrounding areas- with PRN positions available for clinicians who will accept patients for the duration of their stay. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.
