Job summary

Rehab Without Walls Neuro Rehabilitation in Sparks, NV seeks a licensed clinical social worker to perform comprehensive assessments and lead treatment planning with the rehabilitation team.

You will coordinate with families and health professionals, identify resources, and support patient independence in real-world settings. This full-time 32-hour position offers CEU courses and growth opportunities.

Qualifications

• Master's Degree in Social Work or equivalent; active licensure as LCSW.
• Minimum one year in rehabilitation settings with TBI or spine injuries.
• Current BLS/First Aid and CPR certifications.
• Strong communication and ability to work independently and in a team.

Responsibilities

• Perform comprehensive social work evaluations and set measurable goals with the rehab team.
• Identify community resources to support patient independence and recovery.
• Provide direct social work intervention and counseling to patients and families.
• Regularly reassess treatment progress and adjust goals as needed.
• Communicate patient progress to the treatment team and families.
• Collaborate with medical providers on evaluation, progress, and discharge.
• Document and maintain all required clinical and billing records per regulations.
• Adhere to policies, HIPAA, and professional standards.
• Perform other duties as assigned.

Responsibilities

• Conducts comprehensive evaluations, assesses the social, emotional and support needs of patients and their families, and sets measurable objectives and goals that are formulated in conjunction with the rehabilitation team. Adjusts treatment plan to achieve maximum results in independence and/or function.
• Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives. Utilizes special skills and judgment to resolve adverse social and emotional responses connected with the injury/illness.
• Provides direct social work intervention and instructs and counsels the patients and family members in treating and coping with social and emotional responses connected with the injury/illness.
• Regularly re-assesses effectiveness of treatment plan, progress of patient towards goals and the need for modifying goals or treatment.
• Communicates patient’s needs and progress to the treatment team, physician, patient, family members, and others as applicable.
• Consults with medical providers regarding patient evaluation & treatment, progress of the patient & discharge from treatment.
• Observes/documents and reports changes in the condition/status of the person receiving services to the Clinical Coordinator, Program Manager or DCM, attending physician and other treatment team members.
• Maintains and submits evaluation, treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulations.
• Adheres to standards of the Company's Code of Business Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws.
• Performs other duties as assigned.

Qualifications

• Master's Degree in Social Work or Marriage and Family Therapy (LCSW, MSW, MFCC, or LPC)
• Current Social Work or Marriage and Family Therapist licensure/certification in the state(s) of practice required by individual states.
• A minimum of one year of experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or other acute diagnosis.
• Current BLS (Basic Life Support) First Aid and CPR certifications.
• Demonstrates effective and professional interpersonal, verbal, and written communication skills.
• Able to work independently and part of a team.
• Demonstrates strong organization skills and attention to detail.

Additional Job Information

• Ability to lift fifty (50) pounds. Moving, lifting, or transferring of patients may involve lifting of up to 100 pounds.
