Qualifications

Responsibilities

Responsibilities

• Perform comprehensive evaluations, assessing the social, emotional and support needs of patients and their families
• Set measurable objectives that are formulated in conjunction with the rehab team
• Assist physicians and other health team members in understanding significant social and emotional factors related to the patient’s health problems
• Instruct and counsel patients and families in treating and coping with social and emotional responses connected with illnesses
• Participate in care coordination activities and act as a resource to other health team members in identifying and resolving patient needs
• Identify and assist patients and/or families in utilizing appropriate community resources to achieve measurable objectives

Qualifications

• Master’s Degree in Clinical Social Work (LCSW)
• Current Licensed Clinical Social Worker licensure in the state of practice
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
• Current BLS (Basic Life Support) First Aid and CPR certification required
• Ability to lift 50 pounds
• Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
• Duties require fine motor skills, visual acuity, and walking/standing for extended periods
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times

Salary Range: USD $60.00 - $65.00 / Hour
