About the job:

The Licensed Clinical Social Worker, supervised by the Center Manager, plans, organizes, and implements social services for Senior Community Care participants and families. Responsibilities include assessment, treatment, teaching, counseling, case management, advocacy, and acting as a liaison between the Interdisciplinary Team, caregivers, and community agencies.

Required Qualifications:
  • Master's Degree in social work from an accredited school
  • Current license, registration, or certification as per state requirements
  • At least one year of experience working with frail or elderly populations
  • Must pass background check
  • Valid driver's license and transportation
  • Medical clearance for communicable diseases and up-to-date immunizations
Essential Duties:
  • Conduct initial psychosocial assessments for enrollment
  • Perform re-assessments every six months
  • Participate in Interdisciplinary Team meetings
  • Provide ongoing support, counseling, and education to participants and families
  • Coordinate admissions/discharges and facilitate meetings with participants, families, and agencies
  • Support hospice care, grief work, and community resource referrals
