Impact Health PC is seeking a full-time Licensed Clinical Social Worker (LCSW) to provide mental health services to residents of skilled nursing facilities in the Waterville, Maine area. You will conduct assessments, provide individual therapy, and collaborate with a multidisciplinary team during typical business hours.

Active Maine LCSW license, 2+ years of adult mental health experience, and SNF/geriatric experience are required.

Qualifications

• Active Maine LCSW license.
• At least 2 years of clinical experience providing mental health care to adults.
• Experience in skilled nursing facilities or geriatric mental health.

Responsabilités

• Provide person-centered mental health care using evidence-based practices.
• Conduct assessments and provide diagnostic and psychotherapy services.
• Meet with residents individually for therapy sessions.
• Manage a caseload and address residents’ mental health needs.
• Coordinate care with facility staff and Impact Health’s clinical team.
• Collaborate with medical and psychiatric providers.
• Complete documentation within 24 hours of each encounter.
• Follow applicable state regulations and SNF requirements.
