Job summary

Impact Health PC in Bath, ME is seeking a full-time Licensed Clinical Social Worker to provide mental health services to residents of skilled nursing facilities in the Bath area. You will conduct assessments and provide individual therapy, manage a caseload, and coordinate care with the facility staff and Impact Health’s psychiatric NP, therapists, and physicians.

Schedule is flexible within normal business hours.

Qualifications

• Active Maine LCSW license.
• Minimum 2 years of clinical experience with adults.
• Experience in skilled nursing facilities or geriatric mental health.
• Strong communication, assessment, diagnostic, and psychotherapy skills.

Responsibilities

• Provide person-centered mental health care using evidence-based practices.
• Conduct assessments and provide diagnostic and psychotherapy services.
• Meet residents individually for therapy sessions.
• Manage a caseload and address residents’ mental health needs.
• Coordinate care with facility staff and Impact Health’s clinical team.
• Complete documentation within 24 hours of each encounter.
