Job summary

Impact Health PC in New Haven, CT seeks an LCSW to provide psychotherapy and behavioral health services within skilled nursing facilities. You will manage a caseload, conduct assessments, and collaborate with psychiatric providers, facility staff, and an interdisciplinary care team.

Flexibility is valued with autonomy in clinical work, and a supportive team environment is offered along with schedule-friendly hours. 2+ years’ relevant experience and CT license are preferred.

Qualifications

• Active Licensed Clinical Social Worker (LCSW) license in Connecticut.
• 2+ years of clinical experience providing mental health care to adults preferred.
• Experience in skilled nursing facilities, geriatric mental health, hospitals, residential care, or psychiatric outpatient settings preferred.
• Strong assessment, diagnostic, psychotherapy, and communication skills.
• Ability to work independently and collaboratively as part of an interdisciplinary care team.

Responsibilities

• Provide psychotherapy and behavioral health care to residents in skilled nursing facilities.
• Conduct clinical assessments and identify residents' behavioral health needs.
• Manage an assigned caseload within skilled nursing facilities.
• Develop and support individualized treatment plans.
• Collaborate with psychiatric providers, medical staff, facility teams, and interdisciplinary care team.
• Educate and support residents, families, and facility staff as appropriate.
• Complete timely and accurate clinical documentation.
• Maintain compliance with facility, state, and federal regulations.
