Rehab Without Walls Neuro Rehabilitation, an affiliate of BrightSpring Health Services, seeks a Licensed Clinical Social Worker for a full-time role in Houston, TX. The position emphasizes 1:1 in-person therapy with optional indirect work from home, delivering clinical social work within a multidisciplinary rehab team.

Candidates should have a Master’s in Social Work or related field and TX licensure, with 1+ year rehab experience and CPR/BLS certification.

Qualifications

• Master’s Degree in Social Work or Marriage and Family Therapy.
• Current Social Work or Marriage and Family Therapist licensure in the state(s) of practice.
• Minimum one year of rehabilitation setting experience.
• BLS/CPR certifications.
• Strong interpersonal, verbal and written communication.
• Ability to work independently and as part of a team.
• Excellent organization and attention to detail.

Responsibilities

• Conduct comprehensive evaluations of patients and families and set goals with rehabilitation team.
• Identify community resources to meet patient objectives and address social-emotional needs.
• Provide direct social work intervention and counseling to patients and families.
• Regularly reassess treatment effectiveness and adjust plans as needed.
• Communicate patient progress to treatment team, physicians and caregivers.
• Document evaluations, treatment and billing promptly per regulations.
• Adhere to HIPAA, ethics, and company policies.

• We are seeking a Licensed Clinical Social Worker (LCSW) to provide specialized mental health counseling for social and emotional adjustment during rehabilitation from catastrophic injury or illness.
• Support individuals and families as they adapt to disability and lifestyle changes.
• You will provide therapeutic interventions & resource support and navigation that address the patient’s goals
• Knowledge of medical social work and/or neurological injury (TBI/CVA)
• 10+ years of experience preferred

Responsibilities

• Conducts comprehensive evaluations, assesses the social, emotional and support needs of patients and their families, and sets measurable objectives and goals that are formulated in conjunction with the rehabilitation team. Adjusts treatment plan to achieve maximum results in independence and/or function.
• Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives. Utilizes special skills and judgment to resolve adverse social and emotional responses connected with the injury/illness.
• Provides direct social work intervention and instructs and counsels the patients and family members in treating and coping with social and emotional responses connected with the injury/illness.
• Regularly re-assesses effectiveness of treatment plan, progress of patient towards goals and the need for modifying goals or treatment.
• Communicates patient’s needs and progress to the treatment team, physician, patient, family members, and others as applicable.
• Consults with medical providers regarding patient evaluation & treatment, progress of the patient &discharge from treatment.
• Observes/documents and reports changes in the condition/status of the person receiving services to the Clinical Coordinator, Program Manager or DCM, attending physician and other treatment team members.
• Maintains and submits evaluation, treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulations.
• Adheres to standards of the Company's Code of Business Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws.
• Performs other duties as assigned.

Qualifications

• Master's Degree in Social Work or Marriage and Family Therapy (LCSW, MSW, MFCC, or LPC)
• Current Social Work or Marriage and Family Therapist licensure/certification in the state(s) of practice required by individual states.
• A minimum of one year of experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or other acute diagnosis.
• Current BLS (Basic Life Support) First Aid and CPR certifications.
• Demonstrates effective and professional interpersonal, verbal, and written communication skills.
• Able to work independently and part of a team.
• Demonstrates strong organization skills and attention to detail.
