Job summary

Rehab Without Walls Neuro Rehabilitation in Charleston, SC is seeking a licensed clinical social worker (LCSW) to provide counseling within home and community-based neurorehabilitation settings. You will evaluate social, emotional, and support needs and collaborate with the rehab team to define measurable goals.

The role emphasizes autonomy, teamwork, and coordination with healthcare professionals; BLS/CPR certification is required and the position offers flexible scheduling and growth

Qualifications

• Master's degree in Clinical Social Work (LCSW) and current LCSW licensure in the state of practice.
• Minimum of one year experience in a rehabilitation setting serving individuals with brain/spinal injuries or acute diagnoses.
• Current BLS First Aid and CPR certification required.
• Ability to lift 50 pounds; occasional lifts up to 100 pounds with proper safety procedures.
• Moving, lifting, or transferring patients as needed per safety requirements.

Responsibilities

• Performs comprehensive evaluations of social, emotional and support needs of patients and families.
• Sets measurable objectives in conjunction with the rehab team.
• Assists health team in understanding social and emotional factors related to health problems.
• Instructs patients and families in coping with social and emotional responses to illness.
• Participates in care coordination activities and acts as a resource to health team members.
• Identifies and helps patients/families access community resources to meet objectives.

Job Details

• Location: US-SC-CHARLESTON
• Position Type: PRN
• Pay Min: USD $45.00/Hr.
• Pay Max: USD $48.00/Hr.

Responsibilities

What you will do: (responsibilities listed are included but not limited to)

• Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families
• Sets measurable objectives that are formulated in conjunction with the rehab team
• Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems
• Instructs/counsels' patients and families in treating and coping with social and emotional response connected with illnesses
• Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
• Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives

Qualifications

• Master's Degree in Clinical Social Work (LCSW)
• Current Licensed Clinical Social Worker Licensure in the state of practice
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
• Current BLS (Basic Life Support) First Aid and CPR certification required
• Ability to lift 50 pounds
• Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
• Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
