Rehab Without Walls Neuro Rehabilitation is seeking a full-time Licensed Clinical Social Worker (LCSW) for our outpatient clinic in Portland, Oregon. Qualifications

What You'll Do

• Provide psychosocial assessments and behavioral health support for patients and families
• Develop individualized treatment and support plans based on patient needs and goals
• Deliver counseling, education, and resource coordination services
• Assist patients and families in navigating community resources and support systems
• Collaborate with physicians, therapists, case managers, and other treatment team members

What We're Looking For

• Master’s degree in Social Work
• Current LCSW licensure in Oregon
• Strong communication, counseling, and care coordination skills
• Experience working with neurological, rehabilitation, behavioral health, or medical populations preferred
• Passion for helping individuals achieve meaningful outcomes and improved quality of life

Responsibilities

• Perform comprehensive evaluations, assessing the social, emotional, and support needs of patients and families
• Set measurable objectives formulated with the rehab team
• Assist physicians and other health team members in understanding social and emotional factors related to patient health problems
• Instruct patients and families in coping with social and emotional responses to illness
• Participate in care coordination activities and act as a resource to other health team members in identifying and resolving patient needs
• Help patients/families utilize appropriate community resources to achieve objectives

Qualifications

• Master’s degree in Clinical Social Work (LCSW)
• Current Licensed Clinical Social Worker licensure in the state of practice
• Minimum of one year experience in a rehabilitation setting with traumatic brain injury, spinal cord injury, or acute diagnosis
• CPR certification required
• Ability to lift 50 pounds
• Capacity for lifting or transferring patients, including up to 100 pounds, following safety procedures
• Requires fine motor skills, visual acuity, and the ability to stand or walk for extended periods

Salary

USD $37.64 / Hour
