6 Degrees Search is seeking a Licensed Clinical Social Worker (LCSW) in rural Northeast Oregon to join a premier hospital’s behavioral health team. This permanent, full-time role offers four 10-hour days, a supportive environment, and a compensation package up to $88k annually.

The LCSW will conduct psychosocial assessments, provide crisis intervention, collaborate with medical staff, and support post-discharge planning in a hospital setting.

Qualifications

• MSW degree from an accredited institution.
• Current, active LCSW license or eligible for immediate Oregon licensure.
• 1–2 years of clinical social work experience, preferably in hospital/clinic settings.
• Willingness to practice in a rural Northeast Oregon community.

Responsibilities

• Conduct psychosocial assessments, mental health screenings, and diagnostic evaluations for patients and families.
• Provide crisis intervention, short-term counseling, and medical social work services.
• Partner with physicians, mid-level providers, and nursing staff to coordinate seamless care transitions.
• Educate patients and families on disease pathology, coping mechanisms, and healthcare decisions.
• Identify barriers to health, arrange post-discharge care (home health, rehab, equipment).
• Maintain accurate, timely documentation within the EHR for billing and continuity of care.
• Participate in multidisciplinary rounds to discuss complex cases and improve outcomes.

A premier hospital in beautiful Northeast Oregon is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join their expanding behavioral healthcare staff. Key Responsibilities:

• Conduct comprehensive psychosocial assessments, mental health screenings, and diagnostic evaluations for patients and families.
• Provide evidence-based crisis intervention, short-term counseling, and medical social work services to support patients dealing with acute or chronic medical conditions.
• Partner directly with physicians, mid-level providers, and nursing staff to coordinate seamless care transitions between inpatient units, outpatient clinics, and community-based resources.
• Support, counsel, and educate patients and their families on disease pathology, coping mechanisms, and complex healthcare decisions.
• Identify barriers to health and safety, facilitate emergency interventions when necessary, and arrange for post-discharge care, including home health, rehabilitation, or medical equipment.
• Maintain accurate, timely, and secure documentation of all patient encounters within the electronic health record system for billing and strict continuity of care.
• Participate actively in multidisciplinary team rounds to discuss complex cases, optimize clinical outcomes, and champion holistic patient wellness.

Qualifications:

• Master of Social Work (MSW) degree from an accredited institution
• Current, active license as a Licensed Clinical Social Worker (LCSW) or eligible for immediate licensure in the state of Oregon
• 1–2 years of clinical social work experience, preferably within a hospital, clinic, or integrated healthcare setting
• Comfort with and a genuine interest in living and practicing within a rural community setting
• Excellent crisis intervention, communication, and interpersonal skills with a strong collaborative mindset
