West Cecil Health Center is looking to add to their Medical Practice Team! WCHC is seeking Licensed Clinical Social Workers - Certified (LCSW-C), for both Part-Time and Full-Time positions in Conowingo and Havre de Grace, MD. Ideal candidates are passionate about their work, eager to make a community impact, and interested in ongoing development. Work arrangements are mostly remote or onsite.

The LCSW-C is responsible for evaluating, diagnosing, treating, and coordinating services for patients with mental, behavioral, and emotional issues.

Essential Functions of the LCSW-C

• Provide services within the scope of practice accepted for a Licensed Certified Social Worker at WCHC
• Support the mission by serving as a liaison and promoting the health center within the behavioral health community
• Assess psychosocial needs, strengths, and support networks of patients
• Develop care plans and treatment goals with patients
• Establish rapport and advocate for patients
• Provide therapeutic support, individual and group therapy
• Assist Primary Care Physicians with behavioral health concerns
• Help pursue additional funding opportunities to expand community services
