Job summary

Qualifications

• Licensed Clinical Social Worker (LCSW) license valid in Illinois.
• Experience providing brief, evidence-based therapy to individuals and groups.
• Develop individualized treatment plans in collaboration with patients.

Responsibilities

• Conduct comprehensive clinical assessments and diagnostic evaluations to identify patients' behavioral health, emotional, psychological, and psychosocial needs, including urgent and crisis assessments.
• Provide goal-focused, evidence-based therapy (individual, couples, family, and group) with potential for longer-term work when clinically appropriate.
• Develop and monitor individualized treatment plans with measurable goals.
• Maintain timely documentation in electronic health records with HIPAA-compliant practices.
• Coordinate care with psychiatrists, physicians, nursing staff, case managers, and other team members.
