Job description

Responsibilities
  • Provide direct clinical social work services, counseling, discharge planning, and case management to hospital patients and their families.
  • Conduct biopsychosocial assessments and document emotional, psychological, social, financial, and environmental needs, as well as follow‑up referral requirements.
  • Initiate and discontinue 5150 custodial holds within NorthBay Medical Center, Vaca Valley Emergency Rooms, and inpatient units when appropriate.
  • Collaborate with interdisciplinary healthcare staff to manage the psychosocially complex patient across the continuum of care.
  • Formulate a plan of care related to the patient’s strengths and needs, develop a discharge plan, monitor progress toward goals, and identify barriers to meeting the estimated length of stay.
  • Facilitate integration with community‑based resources to optimise patient health, and communicate relevant information to care providers.
  • Document assessments and interventions in the patient’s medical record consistent with system mission, regulations, and policies.
Education
  • Master of Social Work from a school of Social Work accredited by the Council on Social Work Education.
Licensure
  • California BBB Licensed Clinical Social Worker (LCSW); CCS paneling required if working with the pediatric/Neonatal Intensive Care (NICU) population.
Experience
  • Minimum of two years experience within the acute health care setting or community health setting.
Qualifications & Skills
  • Strong clinical assessment and critical thinking skills, capable of independent judgment and clinical decision‑making.
  • Excellent interpersonal and communication skills, both verbal and written, and the ability to convey instructions accurately.
  • Excellent follow‑through and organisational skills, able to plan, set realistic objectives, and meet deadlines.
  • Ability to direct actions of others effectively to accomplish tasks and functions.
  • Ability to work effectively with diverse populations regarding age, culture, ethnicity, and gender.
  • Knowledge of individual, child, and family development, human behaviour, communication, cultural influences, psychotherapy, case management, and the relation between social work practice and medicine.
  • Skilled in working with patients/families coping with loss and grief.
  • Knowledge of child, elder, and dependent adult abuse, domestic violence reporting requirements, and related legislation and laws such as Tarasoff, HIPAA, and others.
  • Familiarity with suicidal behaviour assessment, DSM‑V, legal requirements, and guidelines for psychiatric services—including appropriate use of 5150.
  • Basic proficiency in word processing; experience with Cerner and PowerPoint preferred.
  • Knowledge of acute care, subacute care, long‑term care, home care, hospice interventions, rehabilitation options, county mental health services, and other community resources and requirements—strongly preferred.
  • Ability to conceptualise clinical situations and demonstrate application of theory in case management and psychotherapeutic responsibilities.
Physical Effort

  • Requires standing, walking, stooping, bending, twisting, periods of sitting, and continuous computer keyboard use throughout the workday.
  • Transportation may be required to cover referrals when hospital needs coverage.

Hours of Work
  • Based on business need; schedules may include 7:45 am–4:15 pm or 8:00 am–4:30 pm Monday–Friday; may require weekend or holiday coverage.
Compensation
