Salary Range: $78,000.00 - $95,000.00 Salary/year

Position Summary

Open Door is seeking a Full-time bilingual English-Spanish speaking Licensed Clinical Social Worker for its Family Medical Center in Mount Kisco, NY. The Licensed Clinical Social Worker will report to the Director of Behavioral Health. The dedicated individual will work closely with the primary health care team to provide integrated behavioral health care to improve the patients’ medical outcomes. The licensed clinician will possess excellent clinical skills, provide short-term Behavioral Health care using evidence-based treatment models with adults, children, and families, and establish and implement clinical treatment goals in collaboration with the integrated health care team.

Responsibilities

• Apply high quality, appropriate short-term treatment based on clinical diagnosis and assessment of the patient’s presenting problem.
• Provide short-term treatment modalities at Open Door site including, but not limited to, individual, family, and group counseling.
• Accurately document all clinical interventions, diagnosis, symptoms, presenting problems in the Electronic Medical Record.
• Work with patient to develop treatment plan and goals, based on social, emotional, and health needs of the patient/family.
• Provide crisis intervention and conduct Warm Hand Off assessments as needed.
• Incorporate health and wellness education and referrals to preventive care.
• Coordinate and case-manage plans within agency and with outside agencies, including interfacing with school, criminal justice, medical, and social service agencies.
• Coordinate care with patient’s care team, and communicate regularly with care team.
• Participate in weekly one hour supervision with LCSW supervisor.
• Participate in multi-disciplinary team conferences and service training programs.

Requirements

• Master’s Degree in Social Work
• Current New York State registration and license as a Licensed Clinical Social Worker (LCSW)
• Bilingual Spanish-English
• Minimum of 3 years of experience providing clinical treatment to families and children, preferably in the health care field, in individual, family, group, and crisis intervention modalities
• Excellent clinical experience with mentally ill, substance abuse afflicted population
• Excellent clinical, assessment, and diagnostic skills
• Proficient in the use of computers, and specifically with electronic medical records
• Excellent written and verbal communication skills with a culturally diverse population
• Team-focused collaborator
• Effective functioning in fast-paced, high-accountability environment
• Passionate about providing high quality care to low-income, underserved residents in healthcare setting
• Highly organized, self-directed, and able to multitask
