Job summary

The Floating Hospital Inc. in Long Island City, New York seeks a full‑time Licensed Clinical Social Worker to deliver mental health therapy within the Outpatient Behavioral Health Program.

You will work under the Director of Behavior Health Service with a multidisciplinary team to develop personalized treatment plans and cohesive care paths for patients across ages. Qualifications include a NY LCSW license, minimum two years of clinical experience, and preferred English/Spanish fluency.

Qualifications

• Experience with substance abuse treatment is a plus.
• Experience with EMR systems (Athena preferred).

Responsibilities

• Perform Mental Health Intakes to assess diagnoses, level of care, and create treatment plans.
• Provide ongoing individual and/or group therapy with responsibility for patient care.
• Offer crisis intervention to diffuse conflicts and ensure safety.
• Maintain confidential, accurate medical documentation for patient care and referrals.
• Coordinate referrals to other mental health providers and services as needed.
• Collaborate with psychiatrists and other clinicians for comprehensive care.
• Support health promotion and patient advocacy within the clinic.

Under the direct supervision of the Director of Behavior Health Service, we are seeking a full‑time Licensed Clinical Social Worker to provide mental health therapy for patients its Outpatient Behavioral Health Program.

Essential Duties:

• Ability to work across the age spectrum and working with families.
• Performs Mental Health Intakes for patients to ascertain their diagnoses, determine appropriate level of care, and establish treatment plan
• Provides ongoing individual and/or group therapy, maintaining ultimate responsibility for directing the course of the patient's mental health care and treatment plan within the scope of their expertise
• Provides crisis intervention to diffuse conflicts among patients
• Ensures a welcoming, unbiased, and inclusive environment for all who seek care
• Completes medical forms and documentation related to patient care including any necessary to facilitate transfer, and if indicated, referrals to outside programs or psychiatric clinics
• Encourages health promotion, disease prevention, and health maintenance
• Provides patient advocacy, as appropriate for the site
• Completes documentation including diagnosis, treatment, and billing, thoroughly, accurately, and in a timely fashion
• Makes and/or tracks all referrals to other mental health agencies and providers
• Works collaboratively with specialists for consultation and referral purposes, collaborates with psychiatric providers and medical providers within and across the clinic as needed
• Maintains confidential case files, including documenting requests for confidential information
• Follows the clinical chart review policies and procedures
• May provide workshops for families on communication and other family‑related issues
• Coordinates emergency responses, including a team to minimize patient’s danger to self and others, direct discussion with EMS/FDNY regarding patient’s condition, referral to psychiatric or medical emergency departments and follow‑up while patient is in the emergency department or upon release
• Participates in case‑conference and case management activities
• Utilizes supervision effectively to improve patient care; engages in critical reflection to develop personally and professionally

Requirements:

Background and Education:

• Minimum of two years of clinical experience.
• Fluency in English and Spanish is preferred.
• Possess a valid, unrestricted New York State Licensed Clinical Social Worker license.
• Prior experience with substance abuse issues preferred.
• Substantive knowledge of EMR systems, Athena is preferred.
