Job summary

MyCHN (Community Health Network) is seeking a Behavioral Health Therapist to provide clinical care, counseling, and coordination of services for patients of all ages within a collaborative care team. The role emphasizes conducting assessments, developing care plans, and coordinating with the interdisciplinary team to improve health outcomes.

Qualifications

• Active and unrestricted Texas license as an LCSW or LPC.
• Ability to practice independently.
• Eligibility for Medicare/Medicaid provider participation.
• At least one year of clinical experience in area of practice.
• Strong communication and interpersonal skills.
• Ability to work effectively with diverse populations.
• Ability to handle sensitive situations and complex patient needs.
• Strong collaboration and care coordination skills.
• Commitment to patient-centered care and positive patient outcomes.

Responsibilities

• Conduct behavioral health assessments and develop individualized care plans
• Gather patient histories through interviews with patients and families
• Provide individual, couples, family, group, and crisis counseling services
• Support wellness management and coordinate referrals for specialized care when needed
• Collaborate with providers, psychiatrists, and interdisciplinary care teams
• Educate patients on behavioral health conditions and health-promoting activities
• Ensure timely and accurate documentation of all patient interactions
• Manage follow-up care and ensure timely communication with patients
• Support integrated care coordination across behavioral health, primary care, and community resources
• Perform additional duties as assigned within scope
