Job summary

Insight Hospital and Medical Center- Chicago is seeking a licensed clinical social worker to provide psychosocial services to patients and families across ages, focusing on emotional and social issues stemming from illness and disability.

Under clinical supervision, you will assess, plan, and coordinate care, facilitate support groups, and collaborate with the health care team to promote patient-centered outcomes and discharge planning.

Qualifications

• MSW from an accredited program.
• Illinois Licensed Social Worker in good standing.
• Experience with patients of various ages.
• Ability to maintain confidentiality and handle sensitive information.
• Strong communication and organizational skills.

Responsibilities

• Document psychosocial history, initial assessment, interventions, and discharge plans in a timely, clear manner.
• Reassess when patient health status changes.
• Coordinate with government agencies as needed.
• Assist in development, planning, and facilitation of therapeutic and educational groups.
• Provide psychosocial assessments and counseling to individuals and families within the system and community.
• Provide services in line with NASW Code of Ethics.
• Maintain knowledge of community resources to aid patients and families.
• Consult with health care team on psychosocial management of patient issues.
• Carry out other duties as assigned.

GENERAL SUMMARY:

Under the direction of the Clinical Director, Behavioral Health Services, the social worker provides psychosocial services to patients and families of all ages with particular emphasis on the emotional and social issues resulting from illness, disability, and/or death.

These duties are to be performed in a highly confidential manner, in accordance with the mission, values and behaviors of Mercy Hospital and Medical Center. Employees are further expected to provide a high quality of care, service, and kindness toward all patients, staff, physicians, volunteers and guests.

Essential Duties & Responsibilities:

• Documents psychosocial history, initial assessment, appropriate interventions, and expected discharge plan in a comprehensive, clear, timely and legible manner.
• Reassessment automatically occurs upon notification of significant change in patient’s health status.
• Coordinates with government agencies.
• Assists in the development, planning, and facilitation of therapeutic and educational groups for patients/families.
• Provides clinical social work assessments and services (information, referrals, and counseling) to individuals and/or families within the Mercy Hospital and Medical Center system as well as the general community.
• Provides services to the patients/families in a professional manner in keeping with the philosophy and mission and principles stated in the Code of Ethics of the National Association of Social Workers.
• Maintains knowledge of community resources that will enable patients and families to use the health care system and other organizations to their greatest benefit.
• Provides consultation to the health care team regarding psychosocial management of patient problems or job-related stresses.

Qualifications:

• Master Degree in Social Work from an accredited school required.
• Licensed as a Social Worker in the State of Illinois required.
• Knowledge of patients’ growth and development needs.
• Ability to maintain a high degree of accuracy in carrying out work duties.
• Ability to cope with stress.
