Licensed Clinical Professional Counselor

We are seeking a licensed Clinical Professional Counselor to join our team in Rockford, IL. This is a full-time, permanent position with day shifts (8 hours).

Salary Range: $74,000 - $76,000 per year, plus benefits. The actual pay will depend on your skills and experience. Please discuss with your recruiter for more details.

Responsibilities:

1. Provide evidence-based psychological services, including screening, assessment, and treatment planning.
2. Manage psychotherapy intakes and treatment plans.
3. Collaborate with a multidisciplinary team working with patients in Long-Term Care facilities.
4. Offer individual, family, and group therapy, along with behavioral management services.
5. Adhere to ethical principles and professional guidelines.

Requirements:

• Fully licensed as a Licensed Clinical Professional Counselor.
• Valid driver’s license and reliable transportation.
• Excellent organizational, verbal, written, and interpersonal communication skills.
• Ability to maintain own schedule and utilize Geriatric Notes Tool (GNT) for session notes.
• Maintain professional relationships with patients and facility partners.
• Ability to work independently and as part of a team.
