Job summary

The Jewish Board of Family and Children's Services in New York, NY seeks a Facilitated Intake Team (FIT) Social Worker to perform comprehensive intake assessments for MHOTRS clinics under the CCBHC umbrella. You will gather clinical history, assess social determinants of health, and make treatment recommendations.

This in-person role requires strong engagement skills, crisis stabilization, and referrals, with collaboration across partner programs to ensure seamless access to care.

Qualifications

• MSW or MHC required; NYS license active
• Minimum 3 years of clinical experience in a professional setting
• Bilingual (Spanish/English) preferred but not required

Responsibilities

• Review demographic and clinical information prior to intake to ensure accuracy
• Provide a comprehensive assessment of the client’s social determinants of health needs (mental health, substance use, housing, legal, medical)
• Complete 6–8 intakes per day
• Present a clinical formulation during disposition team meetings
• Make treatment recommendations and referrals based on clinical information
• Conduct crisis evaluations and provide crisis stabilization interventions
• Refer clients to appropriate levels of care within CCBHC and the community
• Assess risk and prioritize high-risk cases for treatment
• Maintain timely completion of intake documents in the electronic health record
• Collaborate with partnering programs to ensure access and continuity of care
• Engage in SBIRT, MI, and related evidence-based trainings as assigned
