Job summary

VNS Health is seeking a Licensed Psychiatric Social Worker to join the Children’s Mobile Crisis Team. You’ll provide in-home behavioral health interventions, perform psychosocial evaluations with a multidisciplinary team, and develop short-term service plans to stabilize at-risk youth.

Travel to client homes within two hours, liaise with community agencies, and support the Program Coordinator. You may supervise Mental Health Technicians and students, and maintain thorough case records in line

Qualifications

• LMSW registration in NYS is required to practice.
• Master's degree in social work from a CSWE-accredited program.

Responsibilities

• Delivers outreach services to mentally ill individuals in the community with in-home mental health intervention.
• Collaborates with team to perform psychosocial evaluations and assess service needs.
• Creates and executes short-term service plans with an interdisciplinary team.
• Responds to dispatch calls and travels to clients' homes within two hours.
• Liaises with community agencies and supports the Program Coordinator as needed.
• Maintains complete case histories and records per program procedures.
• Encourages clients to accept mental health services through family engagement.
• May supervise Mental Health Technician or junior staff Social Workers.
