Minimum Qualifications:
Masters in Social Work from an accredited graduate school.
AR License eligible: LMSW, LCSW, OR 1-year master's level INTERNSHIP within IMSL
OR
Master's degree in Social Work from a CSWE program
Licensed by the state of Arkansas Social Work Licensing Board as an LCSW or LMSW
Additional Information:
Responsibilities:
Administers and interprets brief objective symptom measures;
Provides psychoeducation for patients and their support systems;
Provides medication adherence counseling and disease self-management counseling;
Uses Motivational Interviewing to develop behavioral strategies aimed at symptom reduction/functional improvement;
Provides brief problem-solving cognitive interventions aimed at modifying negative thinking and promoting self-efficacy;
Utilizes Self-Care Plan development and skills training to facilitate disease self-management, improved coping, distress tolerance, stress reduction, and relaxation;
Provides substance use/abuse evaluation; identification of maladaptive coping strategies and development of harm reduction strategies;
Conducts brief behavioral interventions aimed at symptom reduction and functional improvements.
Demonstrates competency in providing cognitive, behavioral, and other evidence-based/evidence-informed interventions to individuals/families and groups with behavioral health, substance abuse, and /or chronic/acute physical health issues in a primary care setting.
Consults with healthcare team members to enhance understanding of the patient;
Provides decision support for treatment planning and assists in the implementation and monitoring of biopsychosocial treatment plans.
Documents activities accurately and promptly in the electronic medical record.
Coordinates care of patients with healthcare team members.
Coordinates with UAMS staff and community to provide clinical referrals to behavioral health providers. Identifies, refers and advocates for patients needing specialty behavioral health services and other services.
Is immediately available to the health care team during working hours in the event of a psychiatric emergency.
Assists in provider, resident, and staff training and education in the integrated behavioral health-primary care model; develops/facilitates evidence-based treatment or psychoeducational groups as appropriate to meet patient/family needs.
Develops and maintains collaborative working relationships with community providers to facilitate good communication and streamline referrals for improved patient care;
Participates in regular staff and other required meetings;
Represents the Service Line at UAMS or community meetings as needed.
Participates in departmental quality improvement efforts;
Participates in community outreach as needed.
May perform other duties as assigned.
